West Portal Dental Care Discount Plan – Exceptions

• Patients with PPO dental insurance are not eligible for this program because their insurance already provides discounted fees. Patients enrolled in HMO dental insurance or Medi-Cal may qualify since our office cannot bill those plans.
• If you choose to pay for the full 12 months and choose to cancel your plan early, all fees for cleanings, exams, and X-rays that were previously included in the plan will be totaled and deducted from the refund or charged to the patient. For example, if you end your membership after four months, you will receive a refund to the credit card on file for the portion of the annual fee remaining, minus one-third of the fees already used.
• If you are on a monthly payment plan and choose to cancel your membership early, all fees for cleanings, exams, and X-rays that were previously included in the plan will be totaled and charged to you. Any services already received will be billed at their regular office fees, and any remaining balance on your account will be due at the time of cancellation.
• Memberships and included services cannot be transferred to another person.
• No additional discounts (such as senior discounts or promotional offers) can be combined with this program.
• Unused benefits do not roll over into the next year. If the plan services are not used within the designated membership or calendar year, no refunds or credits will be issued.
• Discounts only apply to services performed in our office. Any treatment provided by outside specialists we refer you to (such as endodontists, oral surgeons, or orthodontists) is not eligible for plan discounts.
